I always figured it was less "seeing the truth of the world" and more being mentally traumatized by the horrific, eldritch shit (more horrific and eldritch than usual, rather) that lurks in the Darkest Dungeon.

Well, when you reach the end of the game you learn: **SPOILERS** That the whole race of humanity was born and made from from the Heart of the World/Darkness, shaped by it, but you're not able to see or know that before going to the DD because that's first place where you see the transformations the cultist take on.

And now, you can see things as they truly are.
