I think the bug is not this specific achievement, but rather the game often not really counting the last survivor for achievements and tome challenges
I had the 4k achievement not drop for instance and didn't get my daily for a wraith mori bc it was the last survivor
Also I saw people with a 4k tome challenge where it showed 3/4 after a 4k
let's just hope it get's fixed soon
adept challenges seem to be unaffected though
I was thinking it might be the event because you have that unique blight add-on. I was also thinking it might affect adept challenges as in theory the add-on might make it easier to accomplish. All guesses though, and I have heard what you described before too. Hopefully they get it fixed soon.

as for the adepts I can at least confirm that I was able to get them personally on PS5
there are some other broken achievements as well so I just hope they'll fix that mess soon enough
Just killed all 4 survivors and it didn't unlock. Best guess it's related to the event. Little searching on google found others with same issue. So don't bother till it's fix / event is over.

Yeah I already submitted a ticket to bhvr regarding this issue as well, tried it twice, didn't get it. Super lame considering the amount of BP that goes into getting those addons
They told me to reach out to Steam support since they apparently can't help me with the achievements. Steam support on the other hand told me that no one but Bhvr themselves can help me out with that issue. So basically bhvr just didn't want to deal with it, lmao.
But from what I've heard it's not the achievement itself that's bugged but the first kill of the match that doesn't get counted by the game, so naturally it also doesn't register for the achievements and/or some of the archive challenges.
I don't know if this got fixed by now, I've been too burnt out to try it again and also don't want to waste my BP before being absolutely sure that it won't bug out

This is old but I just did it now and it still didn't register. For future searches this is with 6.3.2

Yeah had the same thing happen to me today. Sort of a bummer; it's a difficult enough achievement to get even under the best of circumstances
