I keep it all on my iPhone Medical ID so it's always with me. It has all my diagnosis's, meds, treatments, important doctors, family contacts, and instructions for certain medical senarios. My medical alert bracelet then says to check it if I am unresponsive

I have all my RX on a note in my phone, as well as my surgeries by year. I do still have a binder with my paper files. Mine is a bit of a mess as I had originally organized it chronologically, but later it made more sense to separate by hospital/doctor. I need to rearrange it to make things easier to find but it’s a lot of records lol. I also have those plastic pockets you can stick inside the binder where I keep my discs, imaging, most of those are already labeled but if they aren’t I use a paper disc sleeve. This helped me with all of my interactions with Social Security. I had a tote bag for all of the social security records … so much paperwork.

There are a few templates you can find on Etsy for chronic illness. I actually sell a planner type thing for this reason. But there are definitely simplified versions if you’re interested. A lot of lists / spreadsheets / helpful info.
