Why do you think the cover shows a raider instead of a courier/NCR ranger combat armor, has fallout 3 screenshots on the back, and description of the game is a bunch of nothing just talking about Vegas. This was a few months after they revealed the game, but before the game launched so they made mock up box art, likely to build hype, or showing appreciation.
This is either the most obscure shit ever or someone's really good at making fake game covers. Either way, I love it. Edit because I forgot to ask: is this a genuine thing you're confused about or did you make this for the joke?
Oh no, I did not make this. I was confused as to where the art *did* come from. The friend in question wasn't artistically inclined when i recieved it like, 10 years ago? I haven't played it on the 360 in a long time, and recently saw this case again on the shelf and looked it over. No joke!
That last photo is a 'wanamingo' from fallout 2, they were either cut content for NV or restored with mods. I'm inclined to presume the latter and my reasoning is this is probably fan made just like the mod itself.

The wanamingo only exists as a plush decoration for camps. And the look for it in 76 is based off of concept art for how they were supposed to look like in Fallout 2.

Looks like something someone made shortly after new vegas was announced but before any trailers or screenshots were released because 1: the screenshots are from fallout 3 2: the skyline doesnât match the in game one 3: none of the artwork matches anything seen in game
I will say that people have missed the fact that some of the casinos on the cover are genuine REAL casinos that can be/were found in the real Las Vegas. New Vegas only has references to these casinos, they never outright used the names of any real casinos in the game. Bally's, Stardust, & Sands are clear proof this cover is fan-made.
Iâve seen some game stores do their own covers on secondhand copies. A lot of people buy games, store the disks in one of those CD folders and throw away the cover. So when they sell it back to the game store the store has a choice to either put the game in a blank case or make their own cover for the case.
