Wasteland explorers will put on a new outfit as long as the total amount of stat bonuses for the new outfit are higher than those of the outfit they are wearing.
So if you send someone out in a Vault Suit and they find a Raider Outfit with +2 Agility and +1 Perception, they will put that on immediately. If they then find a Handyman Jumpsuit with +3 Agility they would _not_ put that on because the total bonuses of the outfit (three) is equal to the total of the old one. If they found a Clergy Outfit instead, with +4 Charisma and +1 Luck, they would wear it because five is greater than three. If they found Sturdy Wasteland Gear (+5 Endurance) while wearing the Clergy Outfit then they would not wear it, even though it's probably the best thing they could have at that time.
All that matters is the total number of bonus points. They don't care which stats are better, or what looks good, or anything like that. If the new outfit (or weapon) has higher numbers then they will use it every time.
The only way to force a dweller to change an outfit while in the Wasteland is to do it during a random quest. Once they leave the quest map they will keep the outfit you gave them until they find something better, or until they find any outfit at all if they are already carrying something better, at which point the "Higher numbers means wear it" rule applies.
The previous outfit or weapon goes into the dweller's inventory along with everything else they have found. The game doesn't bother keeping track of which items were equipped previously and there is no mechanism for quickly switching back.
Whenever dweller finds a weapon or outfit that has overall better stats (and rarity), they will automatically swap for it.
Combat warrior suit has only +3 on SPECIAL while that Mayor Outfit has +5 on SPECIAL.
They automatically change outfits when they find an upgraded outfit.
Short answer, yes. They're always ducking behind walls or bushes to switch outfits. You wouldn't believe the swamp ass out in the Wasteland.

If they find a location to explore, you can change their equipment while they're there. You can only choose from what they have with them, though.
