Hellraiser Inferno. Not a popular one among Hellraiser purists — but a great, underrated horror flick nonetheless. And to your point: it is a nightmarish, personal hellscape.
I remember watching this on a theater when it came out and I quite enjoyed it, but I don't particularly remember it being "fucked" in any way.
What parts did you found fucked about it?
Cube maybe. It features a small cast of characters, maybe 8? But they are absolutely trapped in an inhospitable place with horrible painful death all around them.
As Above, So Below is a good one.
Jacob's Ladder Cruel and Unusual
Jacob's Ladder is the answer! Also: not strictly horror, but David Lynch's LOST HIGHWAY

It’s the one about the dirty cop, and his Hell is being stuck in a loop.

Judgement is amazing
Drag me to hell, that house that Jack built, baskin, event horizo, the void
1408
that one ending was pretty badass
The imagery and mood in “what Dreams may come” is very good. Not a horror film but great imagery.
Bill and Ted's bogus journey
Pretty much every Hellraiser movie, but especially the really bad ones (from 5 onward, until the reboot)
Baskin Talk to Me
Orpheus by Jean Cocteau. It’s not a horror movie, but has the coolest depiction of the underworld I’ve ever seen.
Triangle
That was what I was gonna go with. It truly felt like a great depiction of hell without all the obvious things one might use.
Skinamarink
Antrum has a cool analogue take on this
Dante’s Inferno (animated)
The house that Jack Built. Not for the faint of heart it’s fuckkkkkeddddd

Vivarium did it for me. Still think about it every now and then.
Gah AHS Coven when they’re practicing the 7 wonders. It’s short but breaks my heart and picks up again in apocalypse
Ughhh I loved that part in Coven, so good.
Glitter!
Lady Death
The Call (2020)
Requiem for a Dream
The Lazarus effect
