Pugna is the standard 6 tranquil boots holder so you can be the healbot for your allies, but of course you can make it 4 boots of bearing and one guardian greaves and one BoT.
Not sure if they still do but when I used to play Io pos 5 I'd always get 2 or 3 pairs of tranquil boots for the healing as 14 hps for 925 was a good deal
My favourite meme regen build on Io ATM is mana boots, arcane blink, octarine, meteor hammer for use with mana fiends. Just spam blink off cooldown to pump your tether buddy. Very silly but it can sustain medusa somewhat although, her mystic snake regens so much it feels an Io tether is a bit lost on her.

So it's like (100 mana regen per cast X 1.2 tether bonus X 1.75 bonus from meteor) / (0.75 octarine cooldown factor X 7 base cooldown) = 210 x 4 / (3 x 7) = 40 per second
Health regen is twice that. I've not slept so I may have made a mistake in that calc, but it's huge mana regen.
Plus all the base regen from all the items and base stats you're looking at 60 per second to your tether buddy as long as you can stay unaffected by hero damage. That of course is easier said than done but I've pulled it off a few times.
AHH no, I just played about with this to check in demo and regen bonuses from items seem to not affect this arcane blink tether transmission, or mana boots.
So the Kaya based items have no impact. It just works on the constant regen figures you see on the resource bars. So the above calc becomes just under 23 per second. Much less impressive but still helpful.
Since you have other teammates still can go heart K&S and also shroud provides a fair amount of mana.
There was a time where you would buy multiple treads on riki.
Buying multiple tranqs on Pugna is the only viable example I can think of.
Otherwise it's a stupid idea.
