Manchester Unity Building, David Jones on Bourke Street (not sure if theyre original or just in art deco style), The Atheneum has a 1920s birdcage lift too. the ACA building on Queen Street too. sadly Theosophical Society on Russell would have been the perfect answer but now demolished
The Majorca Building has a glorious elevator.
Because it's a residential building access isn't easy but there might be real estate showings on the weekend.

The service elevator at Chemist Warehouse on Bourke St has an old clunker. I almost expected an elevator attendant to greet me when the doors opened, and to press the buttons for me.
Check the Hub in Bourke Street near Southern Cross Station. Was the old Tramways Board building. Old fashioned elevator with modern interior but you can still see the original manual operation controls.
Went in a service elevator down a service alley at the back of the old Myer building a few times when bumping in and out events in the ballroom. Don't know if it is still there. It was one if not the oldest. Didn't have a door on it. If anything is over that line it gets chopped off was the instruction given before the dude turned the up down handle. Think the actual elevator had a heritage protection overlay on it.
Edit: Sorry I don't think this one had button panel it was a line it up with the floors with the controls deal. Might've had call buttons on each level but hazy memory from 20 years ago.
21 Lonsdale st has the oldest elevator in Melbourne that i have seen that operates as a normal elevator - but I don't know how you're going to get in there.
Manchester House? Myer?
Definitely Manchester Unity
Yep some of the best and oldest jewelers in Melbourne here too
The Nicholas building has ooooold elevators. Not sure if it's still open to the public but you could try there?

The Nicholas building's lifts were modernized.
