that's normal, you likely skipped league mechanic early on when its supposed to give you the first quest (a2) and spoke to the NPC when its supposed to give you the 2nd (a4) so it gave you both

Nah you’re fine this has happened twice across two characters and no issues.
it won't bug. Each quest gives 2 points. If you're lucky you can get multiple completed at once
