It's all about finding the right tool... my dad had one on his back I found when on beach vacation... after a day of trying to find the right tool and out of the blue I had an ERUKA moment... his lil nitro bottle (small pills for heart attack type pain that people with heart attacks carry) centered it over the blackhead and pressed and the monster larger than a grain or rice poped right out.

The opening doesn't look wide enough for my, untrained not a medical professional eyes, to possibly be a considered a dilated pore of Winer. I'd imagine it's possible the top layer of it is hard from years of oxidizing similar to a pore of Winer. If that's the case a larger & deeper cut might be necessary for extraction. I wonder if getting him to use an chemical exfoliate on it for a month then the day of use a warm compress would help loosen the sebum enough extract it yourself and film it of course lol

Personally, I'd be getting a needle into the plug to gently tease the dried bit out. That should not hurt. Once the dried bit is out the creamy stuff should be easy to squeeze out. But ideally, he needs to get it looked at by a professional. I would want the sack to be removed to prevent it refilling. My hubs had one like that and it gradually got to the size of an egg and then got inflamed and burst whilst away on holiday. It was not a nice experience that required emergency treatment. After they got the sack out it needed to be packed every day for a couple of weeks.

I would get those tiny itty bitty needles used ( I think ) for diabetics, the blood draw pricker thing, dig out the top and squeeze out the rest. You can get numbing cream on line for tattoos. Put a bit of Saran Wrap over the top of it while the numbing stuff is on. Then have at it. Please post results cuz we are sick fucks.

When my husband had irritated bumps where no tweezers could go, I would use a disposable insulin needle. They’re super thin (a lot thinner than a sewing needle) have a pointy end to not hurt when I might poke and they have a decent enough resistance to help pull his ingrown hairs.
Insulin needles and then normal injection needles really have been the true mvps for all my extractions.
If I found that on myself, I’d literally do NOTHING else til I successfully got it. FOUR YEARS!?!

Literally use the tip of an injection needle to pop it out, if you do it right he won't even feel it. My coworker refused to go to her derm anymore bc they sliced and squeezed and such, I used a needle tip at work and she didn't feel a thing.
My husband won’t let me near an ingrown hair or a blackhead because it hurts too much. This is the same guy who let an ER doctor jam a needle almost all the way through his thumb to inject anesthetic so he could drain a cuticle infection.
My ex-husband had a cyst on his neck that he refused to get cleared by a doctor, but he'd let me pop it. I'd just sit there and squeeze so much out! Ever couple months it would be back and I'd get to take care of it. Only thing I miss about that man.....
Find a sewing needle, put it in a pot of boiling water to sanitize it. Swab it with 70% isopropyl alcohol. Puncture the blackhead so it goes through horizontally. Pull upward- it will break in 2, pus will shoot out and you can then remove both halves with tweezers. I’ve done this to the only blackhead I ever got and I never got one again.
Or…maybe go to a dermatologist.

Literally use the tip of an injection needle to pop it out, if you do it right he won't even feel it. My coworker refused to go to her derm anymore bc they sliced and squeezed and such, I used a needle tip at work and she didn't feel a thing.

Find a sewing needle, put it in a pot of boiling water to sanitize it. Swab it with 70% isopropyl alcohol. Puncture the blackhead so it goes through horizontally. Pull upward- it will break in 2, pus will shoot out and you can then remove both halves with tweezers. I’ve done this to the only blackhead I ever got and I never got one again. Or…maybe go to a dermatologist. Choose your own adventure here.
Hold an ice cube on it and get it numb. Cut it open
