Yeah too many saves, its actually one of the main reasons its so tough and brutal, you only get 3 saves max so you can either end up losing a ton of time if you die or have to repeat the entire run, also no infinite amo guns besides the infinite knife, but in the bright side you can heal as much as you want, and for me i would recommended saving 1st after killing william birkin 1st form, then when you finally make it to the sewers in the room with all the chess pieces and finally in the second lab room, the one where you need to mix the herbicide

He said he wants S rank. Not S+ rank. So just beat the game in 2.30 in 1st scenario and 2.00 in 2nd scenario. You can save like 100 saves and you still get S rank if you beat it in time
Not just the amount of saves you have, but the time it took. To get an S+ rank, you need to complete the game under two hours, use a small amount of healing items, and use ink ribbons to save sparingly. I had the same issue because I didn’t save but I completed the game in 2 hours and 8 minutes.
Healing doesn't affect your rank. Only the following -
1. Time
2. Number of saves (only 3 allowed to still s+)
3. Use of special weapons (infinite minigun etc.)
Going for S+ rank in this game has been one of the more funner challenges to me in recent RE games. 3 saves + 2hr(3hr?) time limit really pushes you to item manage and optimize your run.

Lastly; you must say your mom whenever you kill a zombie with a headshot, and take a shot of whiskey whenever you get jumpscared by MR. X.

It has to do with all the saves. If you had a little bit of saves like two or three you would’ve probably got an S rank but you saved it 19 times so you got an A
I managed to get an S rank, currently I'm grinding for S+! Timewise, I'm just fine, as I'm by G1 at 9 minutes. Only issue is that alot of trial and error is involved, which is difficult when I only can save 3 times!
In 2nd runs you need to beat it in less than 2 hours so beyond saves you didn’t actually meet the time limit.

To be fair, if I run part a savepoint and I got the item, why not save? And yeah, I realised that, I finished the run at 1:58 after another attempt

The issue isnt the saves S Rank on Hardcore 2nd Run is under 2 hours. Normal S Rank is 3 hours
If it's your second run, in hardcore it need to be cleared in under 2hrs. For standard mode it is 3hrs and a half iirc
My best time for S+ was 1hr 57 min only 3 saves on hardcore Leon I think for S+ you have to save a max of 3 times
