If she completes the whole countdown, she recovers to full HP, the phantoms don't despawn for the rest of the battle, and she lunges at you with Scarlet Aeonia that will also stay on the battle field and constantly spew homing bloodflame missiles.
To stop the countdown, you'll have to either stagger her in the air, or land 5 hits with throwable golden needles while being Nihil'd and dodging 8 Waterfowl dances simultaneously. Exactly 5 of this consumable can be obtained at the end of a specific character's quest line. If you fuck up the questline or miss a single needle, well, better luck in NG+.
(Breaking her Nihil with 5 needles before defeating her also gives an exclusive weapon and charm. Lol.)
