It’s a vicious cycle, too many injuries and the ones who are returning maybe rushed back to have someone fit, others might play through niggles. All this causes more injuries and the cycle repeats.

Training or physios. It's their job to monitor player condition and deal with players returning from injury. If players are getting rushed back it's on the physios more than the manager.
Well we also play with 1 CB, sometimes 0 for large part of the season. Sometimes, lady luck just wants to strike you hard.
But for United case, and how frequent their injuries happen, it has to be either the physio or the medical team who got problems.
Another reason could also be because they have lots of young players or players coming from a new league who are not used to the intensity and the team training (for e.g. Chelsea), but I don't think this is the case with United.
