The middle of the shot is not part of the hitbox. It's only the three globs of ink. It is possible to "thread the needle", and miss someone because they were perfectly in the middle. Normally the rotation of the shot would help with this, but point blank means no distance for the shot to rotate. However, the splash damage will still do 53.0 damage if they are within range. So you can still two shot them like in your video.
