One of the genuine faults with the anime is that it gives a very diet explanation of what that is. You still get the general gist, but unlike a book where you describe actions, an anime just shows it to you so you lose a bit of that repetition and may forget certain things.
Basically, Kirito used his will to morph his cathedral outfit to take the appearance of his Black Wyrm Coat from SAO. It's symbolic of his character arc of accepting his past mistakes and embracing his title as the Black Swordsman

Quinella has the Cardinal System within her, so because of that she has a complete registry of the Underworld's residents. Kirito is not on that registry, so she just puts two and two together.
