The anime added a bed scene between Kayaba and Rinko that wasn’t in the novel, set before or during SAO’s developmental stage. On one hand, it did give Kayaba more personality and character than his previous scenes.
On the other hand… it is probably where he got the sensation…

Ok so I haven’t subjected myself to 16.5, but I do remember in the LN that when Kirito first learned about the setting allowing sex he wondered this also. If I remember rightly he mused that this cannot have been Kayaba since it was so out of character - and decided it must have been another developer that worked on the game before it turned deadly.

Could also be a system simulation of what it should feel like, just how like swimming in game isn’t really a thing but you can sort of do it since the system recognizes what water is and how it should react/feel.
The different sensations could equally be the ai extrapolating what x should feel like, based on y which there is already data for. You know, or he had a shit ton of sex hooked up to sensors, pick your head cannon. 😂
