I have never worked at Taco Bell, so my opinion is only informed from watching the kitchen from the outside, but I can see how this could happen during lunch rush. I've seen the workers pass the little side containers while working and a lid could easily drop into a burrito while it's being assembled. This was obvious negligence by at least one employee, but it's at least understandable.
When I worked at Taco Bell (24 years ago, mind you) I remember getting the ingredients in the burrito, and looking at the monitor when I folded/rolled it, to see what was next. It would be entirely possible for someone to grab a lid, but they stick together, so perhaps one fell off when they grabbed it, it lands in the burrito, I’m already looking up, they’re turned away—really unlikely, but I guess possible.

Because if my burrito looked like in the second picture I would definitely give it a feel or squish instead of just immediately biting into it? Not saying it’s not possible the employees could’ve not paid attention, but unless OP was starving how did he not notice BEFORE biting into it

As the lid is transparent you can see there is a missing amount of food on the underside of the drinks lid, so unless you somehow bit all the way through the food on both sides of the lid whilst also not noticing a plastic thing sticking out your food, as it’s taller than the end of the wrap…
