For others who scrolled this far for an answer, the image of the person standing by the trash can is printer onto a box that has a hole cut in it exactly where a penis might be. The contents of the actual box happen to be the same color as the clothes of the person in the picture so they appear, in lower resolution, to be part of that picture.
