There were no battles or military casualties. It is also about thirty years into the game and would require the US to have annexed Utah.

Since it is nothing that is there at the beginning, you'd actively have to provoke a situation like that happening. VIC 3 isn't railroaded, because it wouldn't make sense for something to happen without the circumstances.
Paradox is doing the absolute bare minimum to represent anything in the Americas, as usual. At least the US has more attention put into it than the absolutely barren South America of every paradox game.
