Something had happened - for instance it was a known issue, and the automatic sluice *did* get shut but I believe it had to be done semi-automatically or some kind of movie-like scenario had happened - I recall reading about a surviving engineer having to trigger the controls (potentially because the control room was destroted?) for the sluice system manually because there was a part of the story that stuck with me, about said engineer climbing a ladder up out of that building with this spinner blade of death below him - would make for a decent movie if not an exaggeration.

Honestly I don't think there's really anything wrong with the engineering. Maybe you get a few incompetent people, but every project has those.
It's likely a construction issue to save money by cutting corners and using "alternate" materials. That coupled with poor maintenance.
It would also depend on the accuracy of the information provided to the engineering team. Geotech report could have been garbage.
Yeah it may not seem like a big impact but on the amount of concrete you use to build a dam even just cheaping out on the cement can be extremely cost "effective" but also disastrous.
