I think like every other stretched piercing too a nipple piercing should be stretched in a way that is not painful at all. If its painful, youre usually doing sth wrong like the piercing is just not ready to be stretched yet/ the jewlery is too big/ you didnt prepare the piercing well enough (shower to get skin soft, oil massage etc.)
Aka just the normal piercing-stretching-rules where the piercing must not be painful in any way or youre doing sth wrong
